ISTAP Patient Photo Consent and Release Forms

Thank you for contributing to the work of the International Skin Tear Advisory Panel (ISTAP). In order to ensure that all submitted images of skin tears are handled ethically and legally, we require the completion of specific consent and release forms.

 

These forms confirm that proper informed consent has been obtained from the patient—or their legal representative—for the use, storage, and distribution of their images. By submitting the completed forms, you are granting ISTAP permission to use these images for educational, promotional, and publication purposes, in line with our mission to advance skin tear prevention and management globally.

Required Forms

To submit a photo of a skin tear, the following three documents must be completed and submitted together:

  • Patient or Legal Representative Consent Form

  • Healthcare Provider Confirmation Form

  • Submitter Consent Form

 

All three forms must be completed, signed, and submitted together. This process protects the patient’s rights and ensures that ISTAP remains fully compliant with the terms of the consent provided.

Patient or Legal Representative Consent Form

This form confirms that the patient or their legal representative has granted informed consent for ISTAP to use their image.

Healthcare Provider Confirmation Form

This form must be completed by the healthcare provider and references the patient consent. It verifies that proper consent has been obtained.

Submitter Consent Form

This form is completed by the individual submitting the photo and also references the patient’s consent.
